Check all boxes to agree to all terms for the website. Type the patient’s name as an electronic signature at the bottom of the page.
On the next page, please complete name, email, address, create a password, etc… and check the box one more time to agree to the terms
When you scroll down, there is a place to enter either payment information. Normal cost is $99 but as our patient you will pay only $1. To ensure that reduced pricing be sure to scroll further down and enter a “Special Access Code”. This code is the same as before: “111111”. Then click “Submit and Join”
The next page gives a link to the “Member Area”
From there, you can click on “RAP-TD Form”.
If this is your first time completing the form, please click the “View Step-By-Step Tutorial” Button at the bottom of the video for a demonstration.
Next Click “Add Patient” and enter the patient information
Then you can complete the form electronically for that patient
Step 4: Contact the Office and Book Your Appointment
Call 704-895-9355 to book and schedule your appointment.
Step 5: Join the IADFW (International Association for a Disease Free World)
Click “Login” in the upper right corner and login.
Once you are taken to the Dashboard, click “Join the IADFW” on the left side of the page.
Choose “Option 1”. Read the warning, scroll down and sign and check the yellow highlighted checkbox, then continue.
On the next page, agree to each of the terms by clicking the yellow highligted link to Select All Checkboxes, or check each box individually. Scroll to the bottom and click “I Agree and Continue”.
On the next page, review the contract, scroll to the bottom and click “I Agree and Continue”.
On the next page, BEFORE filling out any address info look for where it says in red “$99.00 LIFETIME ACCESS” look for the link below that which says “Click Here if you’ve been given a discount code” and click it. As a patient, you will receive membership for $1.
On the Discount Code page enter the discount code found in the email you recevied from the clinic and click “Submit Code”.
You will be taken back to the page where you can enter your billing address and phone number.
Scroll down and click the Box next to “Agree to Terms & Conditions”.
You should note the cost is now showing as only $1 for Lifetime access.
Scroll down and enter Credit Card information, then click “Submit and Join”.
Step 6: Watch the Videos
From the Advanced Medicine Dashboard, click “Media Library” on the left side of the page.
Click “DVDs On Demand” and watch the 2 videos appropriate to your appointment.\
Autism must watch “Heavy Metals” and “Autism” prior to your appointment
